Founders
Jonathan Goldsmith
Co-founder
Inspired by the transformative potential of AI and a drive to improve patient care, Jonathan founded Survey Health in 2024. He previously led both corporate strategy and product teams at multiple publicly-traded tech companies and served as a senior consultant in Deloitte’s Healthcare practice, advising major U.S. payors and providers. Jonathan holds an MBA from Columbia Business School and a BA from Emory University.
Ashok Roy
Co-founder
Driven by a mission to improve patient outcomes, Ashok joined Survey Health in 2025. Prior to this, he focused on clinical innovation and value-based care, having served as Chief Medical Officer of Caravan Health, acquired for $250M in 2022, and later of Signify Health, acquired by CVS for $8B in 2023. He earned his medical degree from the Keck School of Medicine at USC, MPH from Johns Hopkins, and MBA from MIT Sloan.
